Because there are slight sexual references giving the opportunity for sexual reference jokes, in this case it was two girls, therefore guys saying they have erections when they don't is within context.

Get fact right, this is not Boku no Pico, it's a scene from Sono Hanabira ni Kuchizuke wo: Anata to Koibito Tsunagi. I know because I've seen the full scene already. Which is basically a whole 30 minutes of girl on girl.

But of course, saying that it comes from Boku no Pico is a joke itself. Whenever someone ask where does a particular scene comes from, you must always answer with Boku no Pico no matter what.
